Please arrive 15-30 minutes prior to your appointment to complete the forms listed below. If possible, please complete these forms in advance by downloading, printing, completing, and bringing them to your visit.

It is your responsibility to obtain any necessary referral prior to your visit. Payment is expected at the time of visit. We accept cash (exact amount for copayment is appreciated), check, travelers check, money order, Visa, MasterCard, American Express, and Discover.

If appropriate, please make copies and bring medical records and test results for the doctor to review and usually keep. Please bring any relevant CT and MRI pictures (we will not usually keep these). It helps if you have organized them and eliminated duplicate records.

Please be prompt and allow time for transportation and parking. Our doctors usually run on time. If you arrive late, we will try to accommodate you, but it may be necessary to reschedule your appointment. If you need to cancel an appointment, please contact our office at least 48 hours in advance. This allows us to schedule other patients who have been waiting.
